Application Process for Distributors and Restaurant Franchisors - Restaurants Class 3(d)

Class 3(d) for Mozzarella Cheese

The objective of Milk Class 3(d) is to promote the use of Canadian Mozzarella cheese by restaurants that prepare and cook pizza on site. The reduced price of milk paid by dairy processors in this class provides these restaurants with access to mozzarella cheese at a reduced price. Class 3(d) for mozzarella cheese is administered by the Canadian Dairy Commission and took effect on June 1, 2013.

Application Process for Distributors and Restaurant Franchisors

Section 1

If you are a distributor or restaurant franchisor that supplies mozzarella cheese to restaurants that prepare and cook pizza on site and if you are not registered in CDC's Special Milk Class Permit Program (SMCPP) you must provide the following information to the CDC. If you are already registered in the SMCPP, please refer to Section 2.

1. Copies of letters of incorporation/registration to establish that you are a legal entity registered at the municipal, provincial or national level.
2. Contact information (names(s) and title(s) of owner(s), business name and address, phone number, fax number, e-mail)
3. Company's financial year end date
4. A list of the restaurants/fresh pizza makers to which you supply mozzarella cheese. Note that your restaurant clients must provide you with their CDC registration number. If they do not have a CDC registration number, they must apply on-line here.

This information must be sent to the CDC in electronic file format to the following email address: applications.demandes@cdc-ccl.gc.ca.

Once the CDC has received all the required information, you will have to sign a Milk Class 3(d) Mozzarella Cheese Agreement and will be issued a Milk Class 3(d) Distributor Permit.

Section 2

If you are a distributor or franchisor currently registered in the CDC'S Special Milk Class Permit Program (SMCPP) and you supply mozzarella cheese to restaurants that prepare and cook pizza on site, you must provide the following information to the CDC.

A list of the restaurants/fresh pizza makers to which you supply mozzarella cheese. Note that these clients must provide you with their CDC registration number. If they do not have a CDC registration number they must apply on-line here.

This information must be sent to the CDC in electronic file format at the following e-mail address: applications.demandes@cdc-ccl.gc.ca.

Once the CDC has received all the required information, you will have to sign a Milk Class 3(d) Mozzarella Cheese Agreement and will be issued a Milk Class 3(d) Distributor Permit.
